OPTIONS
There are 5 Options for CDP®. Please read the following options carefully to see which criteria your qualifications meet. All options require completion of the one day live training NCCDP Alzheimer's Disease and Dementia Care Seminar by an NCCDP Certified Alzheimer's Disease and Dementia Care Trainer CADDCT or an NCCDP Alzheimer's Disease and Dementia Care Seminar conducted by the NCCDP corporate staff.

Notice: While certification promotes and maintains quality, it does not license, confer a right or privilege upon or otherwise define the qualifications of anyone in the healthcare field.
Applications must be submitted within 30 days upon completing the Alzheimer's Disease and Dementia course. If you are unable to submit within the deadline, please contact the NCCDP for further instructions.
OPTION 1
RN/LPN/ LVN/NP or College Graduate (4 yrs) with a degree from an Accredited College or University.
You will need to provide a copy of the Alzheimer's Disease and Dementia Care class certificate provided to you at the seminar.
College graduates: You will be required to provide a copy of your college diploma.
Nurse: You will be required to provide your nurse license or certification #, the agency that licensed or certified you, and the license/certification expiration date.
OPTION 2
GED or High School Diploma.
You will need to provide a copy of the Alzheimer's Disease and Dementia Care class certificate provided to you at the seminar.
OPTION 3
Graduate degree from an accredited College or University.
You will need to provide a copy of the Alzheimer's Disease and Dementia Care class certificate provided to you at the seminar.
Provide a copy of your diploma.
OPTION 4
No license or certification.
Applicants must provide a copy of the ADDC class certificate provided to you at the seminar.
Option 5
Grandfather.
This option is only open for health care professionals who have dementia or Alzheimer's disease certification and or dementia or Alzheimer’s disease trainer certification from another national governing body or university. This option does not require you to take the NCCDP Alzheimer’s Disease and Dementia Care 8 hour live seminar. We do not accept college certificate programs in lieu of certification. Certification is different than a certificate of completion. The certification document you received will have an expiration date and sometimes a certification number.
To qualify you must meet the following criteria:
The following documents must be attached with your grandfather option application:
1. Copy of your college transcripts or degree. Except for nurses, please provide from the state registry the information showing your license or certification is in good standing.
2. A copy of your license or certification such as ADC, LNHA, etc. Except for nurses, please provide off the state registry the information showing your license is in good standing.
3. A letter on company letterhead from your supervisor stating you are employed. Your work experience can include more than one company.
4. Resume.
Corporate Group Discount:
The NCCDP offers a corporate group (10 or more) employee discount if the company is investing in multiple employees certification and when the NCCDP Alzheimer's disease and dementia training took place on the company premises. Ask your CADDCT trainer for the corporate group discount form.

Association Member Discount:
The NCCDP offers a discount to attendees who completed an NCCDP Alzheimer's Disease and Dementia Care seminar at a state, national or international conference and presented by an NCCDP CADDCT trainer. Ask your NCCDP CADDCT® Certified Alzheimer's Disease and Dementia Care Trainer® for the association discount form. You will be asked to upload the form with your CDP application and supporting documents within 30 days of the conference.
